- PERSON- age, occupation, fun activities, where were they
before N.H., place of birth, children, may need to ask
family or social service
- PROBLEMS- diagnosis(old & new)
- PERFORMANCE- MMSE scores, ADL's (feeding, bathing,
dressing, tansferring, walking, assistance)
- PRESSURES- fears, worries
- PHYSICAL- heart, lung, abd, ext., skin, breast (q month)
- PLAN- routine lab, change or D\C meds, NO prn restraints
(Ativan is OK), fall assessments (probable on call duty)
Admit lab: cbc, sma, tsh, ua, drug levels, pre-albumin(<18 ="poor" nutrition)
