The Promotion/Tenure Application Packet
Application materials can be downloaded in MS-Word Format
by clicking the links below. The application packet must be submitted to the promotion & tenure committee
in hardcopy (i.e., as a printed document).
The application packet consists of the four components listed below, plus letters of support.
- The Promotion and Tenure Submission Verification page.
This must be signed & dated when your packet is sumitted.
- The Promotion/Tenure Review Request form.
- Applicants complete the first page of the form.
- The 2nd, 3rd and 4th pages are completed by
the Department Chair, Senior Departmental Faculty, and LSUHSC Administration.
- The Biographical Data Form.
It is important to follow the outline exactly as given (i.e., do not delete or rename any headings).
- The Teaching Portfolio (use this link to download the blank form).
It is also helpful to look at an example of a completed teaching portfolio for help filling out the blank form.
Workshops on promotion and tenure are presented each year by the Associate Dean for Academic Affairs. These
usually commence in May/June. Among other topics, the Teaching Portfolio is explained in some detail. It is highly recommended that applicants attend one of these
workshops.
